All graduating elementary pupils for school year 2004-2005 are eligible to take the PSHS System Search for Scholars if they:

1. belong to the upper 10% of the Grade 5 ranking as certified by their school principal;
2. or have special aptitude in Science and Math as supported by report card and letter of recommendation;
3. are Filipino citizens with no pending application as immigrants in any foreign country;
4. will not be more than 15 years of age by June 2005;
5. are in good health and are fit to undergo a rigorous academic program;
6. are of good moral character;
7. and have not taken the PSHS National Competitive Examination.

Submissions for First Screening

All applicants must submit the following to the PSHS-Registrar’s Office in Diliman, or to any Regional PSHS Campuses, or to the DOST offices and Provincial S & T centers:

1. Fully accomplished application form with four (4) identical recent 1x1 ID pictures;
2. A Php 75.00 non-refundable processing fee;
3. Two (2) long window envelopes with postage stamps (Php 6.00).
